Pathways to Success is a state‑funded, team‑based program designed to strengthen Illinois’ behavioral‑health system for children and youth with complex needs. Launched in January 2023, the program expands access to home‑ and community‑based services that help youth and families build stability, resilience, and meaningful support networks.
Grounded in System of Care values and Wraparound Principles, Pathways centers the voice of the youth and family at every step. Each family partners with a Care Coordinator who guides the process, brings together a supportive team, and connects them with resources that empower progress and long‑term success.
Supports youth who are actively navigating complex challenges, including frequent use of crisis services and coordination across multiple family and youth support agencies. Their Child and Family Team meets each month to keep progress moving and ensure strong, connected support.
Supports youth who are ready to grow with added community‑based services, even if they don’t require the same level of intensity as High‑Fidelity Wraparound. Their Child and Family Team meets every other month to maintain momentum and keep support moving forward.
To participate in Pathways to Success, youth must meet the following guidelines:
Team-based care management facilitating all client services and goals.
Guidance and shared experience from trained peer support specialists.
Provides structured, one-on-one support to help young people build emotional regulation, social skills, and daily coping strategies through goal-oriented relationship building.
In-home clinical support to stabilize families and promote resilience.
Up to $3,000 / fiscal year for essential therapeutic assistance.
Up to $1,500 / fiscal year for tailored client needs.
Relief care for caregivers.
