Community Mentoring

Community Mentoring

Program Description

The goal of Community Mentoring is to provide mental health rehabilitation interventions and supports necessary to assist clients in achieving their rehabilitative, resilience, and recovery goals directly in their own home and community environments.

Moving beyond basic symptom management, Community Mentors work 1-on-1 with clients and families to model healthy coping skills, teach essential life skills, and promote overall stability. Working side-by-side with primary therapists, mentors guide clients toward achieving their personal treatment goals, fostering long-term growth and independence.

  • Service must be recommended in the client's comprehensive assessment.
  • The primary clinician, family, and client collaborate to incorporate mentoring into the Individual Treatment Plan.
